A digital care management solution has been introduced to streamline and coordinate patient support, enabling care teams to monitor and assist patients within their homes and communities. The solution was developed in collaboration with clinical innovation teams and with active involvement from individuals living with chronic pancreatitis, who contributed directly to its design and development.
